How it works

1. Type before you print, in the free Adobe Acrobat Reader, or print the pages and write by hand. Long names shrink to fit, and the tick boxes tick on screen too.
2. Type the person's name once: it fills in on the daily, two-day, weekly and handover pages.
3. Print the daily pages as often as you need and keep them together, newest on top.
4. Write what happened and leave blank what doesn't apply.

• 2 PDF files: US Letter (8.5 x 11 in) and A4, 8 pages each
• Large print: page titles 26 pt, labels 18 pt, the weekly overview's column headings 14 to 16 pt
• The fill-in fields and tick boxes need the free Adobe Acrobat Reader
