The signature lines are blank.

Who it is for

Families and the caregivers who help at home, who keep a written record of the hours and the tasks done together.

How it works

  1. Type before you print, in the free Adobe Acrobat Reader, or print the pages and write by hand. Long names shrink to fit, and the tick boxes tick on screen too.
  2. Type the caregiver's name and the name of the person cared for once: they fill in on the weekly, two-week, monthly and notes pages.
  3. Write the hours for each day, for example 4 or 4.5. The file does no sums: add up the week yourself.
  4. On the pages, Family means the person cared for, or the family member who checks the hours. Both write their initials each day and sign by hand on the printed page.
  5. Print a new page for each week and keep the pages together, newest on top.
