How it works

1. Type before you print, in the free Adobe Acrobat Reader, or print the pages and write by hand. Long names shrink to fit.
2. Type the name once: it fills in on every form page, the wallet cards and notes included. The address, date of birth, phone numbers, doctors, people to call and insurance fill in on both versions.
3. Keep the sheet on the fridge in a clear sleeve, and put a note where it will be seen first.
4. Write the date next to Updated whenever something changes.

Sizes and format

• 2 PDF files: US Letter (8.5 x 11 in) and A4, 8 pages each
• Large print: page titles 26 pt, labels 18 pt (the wallet cards are smaller, to fit a wallet)
• Folded, each wallet card is bank-card size (85.6 x 54 mm) when printed at actual size
• The fill-in fields need the free Adobe Acrobat Reader
• Blank forms, text and lines, with red bands on the cards and notes: no illustrations, no advice
