This planner is not a will or a legal document. It records where your will and other papers are kept.

No page asks for an account number, a user name, a password or a code. Each money, document and digital page asks where to find it instead, and says so on the page.

Who it is for

Anyone who wants their important information and wishes written down in one place, and the family members who help them.

How it works

  1. Type your name once before you print, in the free Adobe Acrobat Reader: it fills in on the cover and on every page with a Name line. The date of birth, the address, the main doctor, the health care agent and the funeral home fill in wherever they appear, too. Every line fills in, the tick boxes tick on screen, and long names shrink to fit.
  2. Or print the pages and write by hand.
  3. Fill in the sections in any order, a little at a time, and write the date next to Updated whenever a page changes.
  4. Keep the pages in a three-ring binder, and tell one or two people where the planner is kept.
