How it works

1. Type before you print, in the free Adobe Acrobat Reader, or print the pages and write by hand. Long names shrink to fit, and the tick boxes tick on screen too.
2. Type a name once in each form: it fills in on that form's name lines. The five forms do not share fields, so a name typed in one form is typed again in the next.
3. The contents page and the bookmarks jump to each form. To print one form, type its pages in the print window. The medication record prints sideways (landscape), and its day boxes are for a pen.
4. The timesheet is a blank record-keeping form. It is not an official timesheet for any state, Medicaid, insurance or agency program. If the care is through a program, use that program's own form and check what it requires.

Sizes and format

• 2 PDF files: US Letter (8.5 x 11 in) and A4, 45 pages each, with all five forms
• Large print: page titles 26 pt, labels 16 to 18 pt, column headings 14 to 18 pt (smaller on the wallet cards and in the medication record's 31-day grid)
• The fill-in fields and tick boxes need the free Adobe Acrobat Reader
• Blank forms, text and lines only: no illustrations, no drug information, no doses, no advice. The checklist is for your own household's routine and is not a care plan
